Simply contact us (phone or fax) and we will be glad to contact your patient and schedule him/her. We offer flexible scheduling and work to accommodate any special needs. We will contact your patients and if we have not been able to successfully reach them within three business days in regard to setting up an appointment we will send you notification by fax. 

We will promptly fax a copy of a letter acknowledging their first visit/session and the Nutritional Assessment & Recommendations for any referred patients so that we can coordinate recommendations as necessary in order to provide the most comprehensive medical nutrition therapy.
